• Anyone diagnosed with any form of cancer who is in active treatment as certified by an oncology health care provider.
• Meets the eligibility guidelines of 400% of the Federal Poverty limits* noted below, and provides proof of income:

* Note: these amounts are 400% of the 2013 Federal Poverty Level as defined by the United States Department of Health and Human Services for all states except Alaska and Hawaii.

Note: for children under 18, documentation showing the income of the responsible parent or guardian must be provided.

Note: We will consider all applications received. Eligibility guidelines will be followed in the event of a large number of applications.

How do I apply?

The following steps MUST be completed in order for an application to be considered:
 

1. You MUST speak with a PNK representative at 1-855-PNK-CURE to complete a brief interview. Note: our hours of operation are 9 a.m. to 5 p.m. Eastern Time, Monday –Saturday.

2. If you are eligible for financial assistance, we will mail or email you (whichever you prefer) an application. You MUST submit a completed financial assistance application.
 

Please complete each section of the application; do NOT leave any of the required fields blank as your application will be automatically disqualified.

Note: Please be aware that submission of a completed application is not a guarantee of receiving financial assistance from PNK Cancer Treatment Foundation. Funds are limited and based on eligibility and availability.
